Forms are a simple but extremely powerful and friendly way to return information provided by the user of a page in a web site.

The user fills in the information in the form and when they click Send, then this is sent to the site owner as a simple e-mail. Programs are available to present this information properly formatted in a Word document or add it to a database.

The form shown is a simple contact form at Independent Mediation, which as the name suggests is a company speciallising in different forms of mediation.

The form also asks for client details and if any brochures are required.

This form is mechanised using FormMail and has been setup so that all e-mail addresses are hidden. This means that the form can't generate any spam.

 
In addition to this simple contact application, forms have a large number of uses :-
